Each school year, all students who will participate in any middle school sport, will need to complete the following steps prior to try-outs. If you do not have these completed prior to try-outs, you will not be eligible to participate.

• Current Physical
  • Students can go to their own doctors or to walk-in clinics for physicals.  This is not the FHSAA form, it is just a regular physical form.
